While Allendale Dental is happy to accept and submit your claims, you are ultimately responsible for all fees. We will make the best effort possible to inform you of any co-payments you have. However, there are many different insurance companies out there and each has several different plans; sometimes even our best efforts are not enough to get all the accurate and pertinent information. We accept most insurances and participate with several. To check if we participate with your plan, please call us during our regular business hours. We do not accept any HMO/DMO plans or Medicaid.
